What to bring to your appointment
Please bring the following to your doctors appointment:
- Medicare, DVA, Pensioner or Concession Card
- Private Health Insurance (if applicable)
- List of any medications, allergies or recent test results
- List of any questions you may have for your doctor
- New patients – Arrive 10 minutes early to fill in a new patient and medical history form.
